Parking Fees
Parking is available at and near the hotel at different rates. NHLBI and
NCQA do not validate or reimburse parking fees.
Conference Fees
The fee for attending the conference is $325. The registration fee covers
all conference materials, an opening reception, two continental breakfasts,
morning and afternoon refreshment breaks, and two networking luncheons. Payment
in full is required to process your registration. To ensure proper credit,
please enclose a separate registration form for each participant. Please note
that third-party checks will not be accepted.
Discounts
Early Registration Discount
Individuals registering by May 1, 2001, are eligible for the discount early
registration rate of $225. Full payment of the $225 (early bird) fee is
required at the time of registration. A separate registration form should be
submitted for each participant.
Team Discount
Three or more individuals from the same organization registering at the same
time are eligible to receive a 15% discount off the regular $325 conference fee
($276). Please note that a separate registration form is still required for
each participant. This team discount may not be combined with any other
discount.
Federal, State, and Local Government Discount
Employees of Federal, state, and local governments are eligible to receive
either the early registration discount off the regular $325 conference fee
($225) or a 25% discount off the regular $325 conference fee ($244) after May
1, 2001. To receive this discount, please submit a letter on agency letterhead
requesting the government discount at the time of registration.
Room Rates and Reservations
Room Rate: $184 per night subject to state and local taxes.
Hotel cut-off date for conference room block is May 4, 2001.
Reservations: Room reservations can be made by calling the
Omni Shoreham Hotel directly at 202-234-0700 or by calling the Omni
Reservations Center at 1-800-THE-OMNI (1-800-843-6664). Reservations can also
be faxed to the hotel at 202-756-5145.
Airfare Discounts
Conference registrants are eligible to receive a discount off regular coach,
business, or first class airfares on United Airlines, American Airlines, and
USAirways flights. To arrange for these special air discounts, call the NCQA
Travel Desk toll free at 877-700-5070, Monday-Friday, 8:30 a.m.-5:30 p.m.
Eastern Time.

If you have special needs, please contact NCQA Customer Support by telephone

e-mail at customersupport@ncqa.org. To ensure
that your needs are accommodated, please contact NCQA prior to May 25, 2001.
Refunds and Substitutes
Registrants canceling prior to May 25, 2001, are entitled to a registration
fee refund, less a $50 processing charge. If you are registered for the
conference and find yourself unable to attend, you may designate a substitute
registrant from your organization by submitting a written notification of the
substitution prior to June 3, 2001. Please ensure that this notification
provides the full name of the original registrant and the name, title,
organizational affiliation, and the telephone number of the replacement.
